For your convenience, registration for all CYS programs is handled at the reception desk at CDC in Building 847. All necessary forms will be provided upon registration. However, at the time of enrollment the following must be presented to our clerks:
- Your child's current immunization.
- Your child's shot record.
- For children over 12 months old, the results of the last tuberculin (TB) skin test. The TB test must be current within the six months of initial registration.
- A current LES and/or pay stub (this will be helpful when filling out forms).
