Claims

Full Replacement Value (FRV) and Claims Process

Most government household good shipments picked up after 1 November 2007 are covered by the Full Replacement Value. The claimant must still submit the DD Form 1840/1840R. Use the front of DD Form 1840/1840R to notify the mover of any loss or damage you find at the time of delivery. If further loss or damage is discovered after the transportation provider departs, use the reverse side of the form, DD Form 1840R. You must file the DD Form 1840/1840R with the carrier within 70 days from date of delivery. Completing and submitting the DD Form 1840/1840R does not constitute filing a claim. A claim is separate and distinct from these forms. Your local legal office can assist you on how to complete and how to file DD Form 1840/1840R and your claim.

The claimant has 9 months from the date of delivery to file a claim with the carrier. The carrier should give the claimant a toll free number or email address to facilitate with the claims filing process. If the claimant does not have this information, he/she must contact the AF Claims Service Center.  If after 9 months, the claimant is dissatisfied with results of filing a claim with the carrier, he/she may file with the AF Claims Service Center.  Claims must be filed within 2 yrs from the date of incident and depreciation is applied. Remember, if you elect to file with the Air Force, do not cash any checks/payments received from the carrier.

Household Good and personal property claims for Minot AFB are filed on-line with the Air Force Claims Service Center. You may contact the Air Force Claims Service Center at 1-877-754-1212 for help with all paperwork. Remember to file your DD Form 1840/1840R with your carrier within 70 days from date of delivery.

If you do not file your 1840/1840R with the carrier, you may file it with the local legal office or the Air Force Claims Service Center within 70 days from the date of delivery.

Defense Personal Property System (DPS)

Program became effective for some bases between 18 November 2008 and all CONUS bases February 2009.  If your move was conducted through the DPS system, you will receive a Form 1850. Report loss or damage via DPS within 75 days via the website listed on the form 1850. Through the DPS website you can file and see if claims are settled while also checking on the status of your shipment while it is enroute.

The DPS website helpline phone number is 800-462-2176 option 5 or 618-220-7332 or DSN 312-770-7332.
